Neurosurgery


Neurosurgery deals with the detection and surgical treatment of diseases, malformations and injuries of the central and peripheral nervous system. Maintaining the nervous system’s functions during therapy requires knowledge, skill and the right instruments. inomed develops products that make this challenging work easier.

Intraoperative neuromonitoring (IONM) is a key tool in this context. IONM is used to continuously monitor important functions of the nervous system during neurosurgical interventions, thus reducing the risk of postoperative restrictions for patients.

With continuous dynamic mapping, neuromonitoring enables the resection of tumours based on the individual functional anatomy and thus supports the functional preservation of brain areas, connected nerve pathways, the spinal cord and peripheral nerve structures. During neuromonitoring, the surgical team receives continuous feedback on the functions in the surgical field by means of neurostimulation and can immediately adjust the surgical strategy in the event of neurological changes.

During surgical interventions in the cerebellopontine angle and the posterior cranial fossa (e.g. resection of vestibular schwannomas), intraoperative neuromonitoring helps to localise and test the function of the cranial nerves. EMG, MEPs and AEPs can be used in parallel.

During surgical interventions on the spine, e.g. in the case of spinal tumours, which could lead to damage to the spinal cord and associated deficits and paralysis, or corrective surgery for scoliosis, it is mainly the ascending and descending spinal cord pathways and the vascular supply that are monitored by recording SEPs and MEPs. EMG stimulation can also facilitate the insertion of implants.

Which monitoring device is best suited for which application in neurosurgery?

For complex neurosurgical interventions, e.g. supra- and intraoperative tumour resections, neurovascular surgery, spinal cord tumours and spinal stabilisation, a multimodal device from the ISIS IOM family is best suited. Depending on the working position and equipment, the ISIS Xpert or ISIS Xpert plus may be the best solution. As a portable version, the ISIS Xpress is the first choice for flexible working at different locations.

For motor mapping and awake surgery with speech mapping, the C2 Xplore Cortical is the obvious choice as a compact device with a high level of user-friendliness.

For research projects that require complex stimulation programmes, the Neurostimulator with standalone software is the perfect solution.
